In Gawande's clinical and moral thinking, prescriptions are not so much a function of a specific illness. His patient cares about the diagnosis, and talking about it is a bonus. No better understanding of medical terms could be achieved, nor could patients and physicians learn to trust one another, if physicians held opinions about reconstructive surgery or biological therapies without first getting to know patients in their homes. Nor does Gawande present his patients with treatment plans. Rather, he presents medical problems; he is not their doctor.
The regrets at stubbornness, the guilt over allowing harm to happen, the horror at having hurt other people - these and others are the cruxes of ethics. Gawande's words are alive with this sense. But his richness of language and his perfectly plausible impersonality compel his reader to contemplate a very different sort of ethical practice. He asks us to take responsibility for the moral issues of society, and he exhorts us to develop a moral practice as holistic and humane as that which he asks of physicians. 7211a4ac4a
